Our office is pleased to process participating network insurance claims—we will make every effort to ensure you receive the maximum benefits available to you under your individual insurance policy.

With numerous insurance companies and frequent changes to dental policies, verifying exact benefits can be challenging. Any pending treatment with another dental provider may affect the benefits available at the time services are rendered. While we make every effort to provide an accurate estimate of your out-of-pocket costs, estimates are based on the insurance information available to us at that time. We cannot guarantee coverage, benefit amounts, or claim payment, as all determinations are made by your insurance carrier.

Please take time to review and understand your dental benefits and policy information. Your insurance policy is a contract between you (and/or your employer) and your insurance company. Coverage, limitations, exclusions, and payment decisions are governed by the terms of your specific plan.

Please note: some “in office” procedures may require the necessity of processing pathology samples with an outside lab, with sample pick-up and facility processing. Charges will likely be processed through your insurance payer and may result in direct charges from the facility itself.

Payment for treatment is due on the day of service, including all fees, insurance deductibles, and co-pays, regardless of your insurance coverage. We accept a variety of options for payment including: cash, credit/debit cards, Cherry, CareCredit, and money orders. We have a 4% processing fee for all credit card transactions. However, we regret to inform you we do not accept personal checks.
